Insurance Forms
Our office staff will gladly file insurance claims for you. Please remember that insurance is a contract between you and your insurance carrier and that you are ultimately responsible for payment of your medical bills.
Referrals
If you are a new patient to our office and your insurance plan requires a referral, it is your responsibility to obtain it from your primary care physician. Our managed care representatives will assist you in obtaining follow up referrals.
Co-pays
Co-pays are due at the time of your visit.
